2013 Energy Assistance Program, please call the number above to
leave your name, energy bill account number, phone number, and your
city of residence.
Eligibility Requirements:
- Picture ID
- Social Security cards for everyone in the home
- Most recent gas bill with the payment coupon still attached OR a shut-off letter w/name, address, and account number
- Proof of total household income for last 12 months
• ex. W-2s, paystubs, letters from Social Security, DHS Budget Breakdown, etc - Bridge Card, Proof of SER assistance, and Proof of Medicaid (if applicable)

Valuable program for Nursing Home Residents
Please help The Information Center spread the word about a valuable program funded by the Michigan Department of Community Health.
The Nursing Facility Transition program is designed to assist persons in a nursing home explore options of returning to the community. Individuals may transition either to a private residence, an assisted living facility, or a licensed adult foster care home, depending on the specific needs of the individual. A nurse and social worker will arrange a meeting in the nursing facility to discuss available options.
What Are The Eligibility Requirements?
- Current Medicaid Recipient or eligible for Medicaid and residing in a nursing facility
What Services Are Available?
Transition services are available to address specific individualized needs using a person centered planning approach. According to MDCH guidelines, services/resources may include the following, dependent on needs:
- Security Deposit
- Furniture/Household goods purchases (Excludes TV’s and electronics such as DVD players)
- Groceries
- Utility deposits/hook-up fees (Excludes Cable)
- Housing assistance
- Home modifications
- Moving expenses
- Cleaning
- Additional services may be available to facilitate a successful transition
The Information Center is also a MI Choice (Waiver program) agent. If the individual meets the current guidelines for that program, additional services may be available to assist after the transition is complete on an ongoing basis. If the individual does not meet the MI Choice (Waiver) guidelines, efforts will be made to identify community resources as needed to address individual needs.
